RN Case Manager II
Milwaukee, Wisconsin
Position Summary: The RN Case Manager II will be working with Medicare/Medicaid patients to act as a clinical resource. The duties include:
– Identify needs through the completion of health screens
– Review requests for specialized services, such as private nursing
– Liaison between members, providers, and pharmacies to review referral requests
– Attend medical appointments with members to ensure compliance and follow up care
– Serve as clinical consultant for the service coordination team
Hours for this Position: M-F 8-5,

Required Skills
What We Look For:
- Must be an RN
- 1-2 years’ experience in hospital setting + sub-acute or home health care experience
- Valid Driver License
