Position Summary:
The RN Case Manager position will be reviewing hospital inpatient charts to determine length of stay, discharge planning, etc.

Hours for this Position:
Direct Placement/ Full Time, Monday- Friday 8am-5pm

Required Skills

What We Look For:

• Must have clear/ active Registered Nurse, RN license
• Knowledge/Experience: Must have at least two years clinical nursing experience.
• Experience with California Children’s Services and/or pediatric experience preferred but not required
• Previous case management experience preferred

Responsibilities

More Insight of Registered Nurse /RN Case Manager Daily Responsibilities:

• Provide nurse case management by reviewing admissions through on-site and telephonic review to ensure medical necessity and appropriate level of care.
• Utilization Review and Concurrent Review
• Review and audit patient charts through on-site hospital visits.
• Act as clinical resources to referral staff and make appropriate referrals.
• Provide patient and provider education.
• Data enters assessments, authorizations and reviews into the system.
