Case Manager, Concurrent Review

Position Summary:
The Case Manager/ Concurrent Review RN position will be reviewing hospital inpatient charts to determine length of stay, discharge planning, etc.
Hours for this Position:
Full time, Monday- Friday 8am-5pm

Required Skills
What We Look For:
- Must have clear/ active Registered Nurse, RN license
- Knowledge/Experience: Must have at least two years clinical nursing experience.
- Experience with California Children’s Services and/or pediatric experience preferred
- Previous case management experience preferred

Position Responsibilities:
- Provide nurse case management by reviewing admissions through on-site and telephonic review to ensure medical necessity and appropriate level of care.
- Utilization Review and Concurrent Review
- Review and audit patient charts through on-site hospital visits.
- Act as clinical resources to referral staff and make appropriate referrals.
- Provide patient and provider education.
- Data enters assessments, authorizations and reviews into the system.
