Behavioral Care Coordinator

Casa Grande, Arizona

Position Summary:

Position Purpose:
Conduct screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment.

 

Knowledge/Experience:

Must be an RN At least 2 years of experience in managed care and/or behavioral healthcare setting. Practical knowledge of Medicaid and Health Plan administration.

Knowledge Worker: Critical Thinking/Execution, Adaptability/Flexibility, Communication/Relationship Development, Technical and Professional Knowledge

 

Position Responsibilities:

  • Identify special needs members through the completion of health screens and other resources.
  • Educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices.
  • Coordinate behavioral health treatment with PCP’s and behavioral health providers.
  • Participate in the development and modification of care plans in conjunction with member, member’s family and managing physician to include, monitoring for treatment compliance and assisting with problem solving in breaking down barriers to treatment to include referrals to community resources.
  • Coordinate urgent care evaluations when necessary.
  • Assist members with locating providers and scheduling appointments as necessary.
  • Assist with development of plan specific literature and education materials in conjunction with medical director and corporate oversight.

 

 

Hours for this Position: 8am – 5pm

Required Skills

What We Look For:

  • Must be an RN

Responsibilities

More Insight of Daily Responsibilities: 

  • 100% telephonic
  • This position has no direct member/patient contact
  • They will be providing technical assistance/coaching to both behavioral and physical health care providers on coordinating care for members.
  • So they will be accessing the data the CM’s put in Truecare and the discussing/coordinating the services need for members.
