Field Case Manager / Registered Nurse
Home Health RN Case Manager— up to $89,440/year
HealthCare Support is seeking an independent Registered Nurse (RN) to join a mission-driven team as a Home Health Case Manager in the St. Paul–Minneapolis Metro Area. This full-time, salaried role offers autonomy in the field and the satisfaction of supporting clients in their homes—backed by a collaborative, non-micromanaged team. Territory:
- Twin Cities Metro Area — patient visits are prioritized closest to your home base
Schedule:
- Monday – Friday, 8:00 AM – 4:00 PM
- Flexible based on field schedule with a paid one-hour break
- No on-call or weekends
Compensation:
- $71,884 – $89,440/year (based on experience)
- Salary-exempt position, not pay-per-visit
- IRS mileage reimbursement

Daily Responsibilities:
- Conduct home visits to assess clients, develop care plans, and oversee caregiver services
- Collaborate with a supportive team that manages scheduling and paperwork while you focus on patient care
Qualifications
- Active RN license in Minnesota
- Valid Minnesota driver’s license
- Experience in home health, home care, or other field-based nursing roles
