Director of Case Management— Madison, WI
HealthCare Support is seeking an experienced RN or Respiratory Therapist or Social Worker (with a Masters Degree) to join a mission-driven team as a Director of Case Management in the Madison, WI Area. This full-time, salaried role offers you the opportunity to over see staff, work with multiple different departments and help make a direct impact on patients experience. If you’re looking for a position that is focused on education, advocacy, and patient experience, this is it!
Schedule:
- Monday – Friday, 8:00 AM – 5:00 PM
- Weekends – on an as needed basis
- No on-call schedule
Compensation:
- $100 / year – $110K / year (based on experience)

Daily Responsibilities:
- Lead and manage the case management team, ensuring compliance, quality standards, and effective discharge planning for complex patient cases.
- Coordinate patient care and transitions by overseeing case plans, facilitating interdisciplinary communication, and driving process improvements for optimal outcomes.
Qualifications
- License as a RN, Respiratory Therapist or Master’s in Social Work
- Minimum three (3) years of medical case management or similar experience
- Must have hospital case management experience
- 2+ years of leadership experience within case management
