HealthCare Support is actively seeking a Clinical Case Manager – RN / LCSW to fill an opening with a Fortune 500 Company in Charlotte, NC and Rocky Mount, NC.
Daily Responsibilities for Clinical Case Manager – RN / LCSW:
- M-F 8AM-5PM
- Managing a case load for healthcare members with long term care needs.
- Partner with community resources for patient care.
Required Qualifications for Clinical Case Manager – RN / LCSW:
- Unencumbered NC RN or LCSW license
- 2+ years of Care Management experience
Preferred Qualifications for Clinical Case Manager – RN / LCSW:
- Home health experience
- Long term care experience
- Discharge planning
