RN Case Manager in Sebastian, FL
HealthCare Support is actively seeking a RN Case Manager in Sebastian, FL to fill an opening.
Daily Responsibilities for RN Case Manager in Sebastian, FL:
- 1. Works with the providers and clinic staff to identify patients at high risk.
2. Supports longitudinal care of the patient with chronic care conditions by:
a. performing assessment of health conditions
b. performing medication reconciliation
c. conducting Motivational Interviewing and Self-Management Goal setting
d. providing patient education
e. creating referrals to appropriate agencies and resources
3. Supports transition of the patient with chronic care conditions from inpatient to outpatient setting, by:
a. performing assessment of transitional needs
b. performing medication reconciliation
c. establishing and reviewing contingency plan
d. providing patient education
e. assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
4. Coordinate with providers to establish or update plan of care
5. Performs accurate and timely documentation in the electronic medical record
6. Participates in daily huddles and Patient Care Coordination (PCC) meetings
7. Prepares accurate and timely reports, as required, for weekly meetings
8. Maintains continued competence in nursing practice and knowledge of current evidence based practices.
9. May perform clinical tasks within their scope of practice
10. Performs all other related duties as assigned. - Shift: 8:00am – 5:00pm: Monday – Friday
Required Qualifications for RN Case Manager in Sebastian, FL:
- Bachelor’s degree in Nursing or an Associate’s degree in Nursing with 4 or more years of experience
- Registered Nurse with an active and unrestricted license to practice in Florida
- Current BLS certification
- 2 or more years of experience in a physician’s office, clinical or hospital setting
- Previous case management experience preferred
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
- Excellent verbal and written skills
- Strong interpersonal skills
- Ability to interact productively with individuals and with multidisciplinary teams
- Proficient computer skills to work efficiently with electronic medical records
- Excellent organizational and prioritization skills
Preferred Education, Experience & Skills
- Experience related to patient education and/or motivational interviewing skills and self-management goal setting
- Fluent written and verbal skills in English.

Pay Details: $44-47/hr
