Lead Dementia Care Navigator (Remote)

HealthCare Support is actively seeking an experience Lead Dementia Care Navigator to fill an opening with our prestigious healthcare client. 

Daily Responsibilities for a Lead Dementia Care Navigator:

  • The Care Navigator will call caregivers who are signed up to provide support for families caring for loved ones with dementia.
  • Handle complex cases. 
  • Supervise other Care Navigators.
  • Will be responsible for leading Case Conference Meetings.
  • 90 day review with providers. 
  • Work hours: will work 4 10-hour shifts between Monday-Friday, will have a set 4 day schedule. Hours are 9:30am-8:00pm EST (30 minute lunch). If they would like an hour lunch, they can do a 9:00 EST start time. 
  • Full time right to hire

Required Qualifications for Lead Dementia Care Navigator:

  • Associates or Bachelors Degree in health-related field
  • 2+ years of professional experience working with patients who have dementia.
  • At least 2-3 years of lead/supervisor/management experience.
  • Needs to be in a healthcare setting, preferably overseeing a team that does care coordination

    Looking for LPN’s, Social Workers, CHW’s, Care Coordinators, Mental Health experts

Pay Details: $27.00 – $31.00 an hour
