Field Nurse Practitioner / Physician Assistant
Healthcare Support is actively seeking a Field Nurse Practitioner / Physician Assistant for a permanent, full-time role with a well-respected healthcare system here in Casa Grande, AZ.
Pay Details: $10,000 Sign-On Bonus!
- Up to $10k Relocation Bonus if needed
- $140,000 – $150,000 a year (based on experience)
Shift: Monday – Friday | 8a-5p | No on call!
Daily Responsibilities for the Field Nurse Practitioner / Physician Assistant:
- Provide high-touch primary care to Medicare Advantage patients with chronic kidney disease through home visits and telehealth, working within an interdisciplinary team that includes a Social Worker and Care Navigator.
- Manage comprehensive patient care including assessments, care planning, medication management, and coordination across care settings, with flexibility to create a personalized schedule and travel plan.
- Ideal candidates have experience in geriatrics, primary care, or nephrology, and demonstrate strong soft skills, adaptability, and a passion for serving complex, chronically ill populations.
Required Qualifications for the Field Nurse Practitioner / Physician Assistant:
- Licensed APN or Physician Assistant in the state of Arizona
- 2+ years of experience as an NP or PA with prior experience in Primary Care, Nephrology, or Senior Care
- Must be able to pass background check, credentialing, drug screen, and vaccination requirements
- Reliable transportation and verifiable good driving record
- Experience working with community resources, and comfortable interacting with patients and care teams both telephonically and in-person
