Senior UAS Assessor -Bronx Full-Time & Part-Time
$110–$112K (FT) | $56.41/hr (PT Max) + Bonus + Mileage
We are actively hiring Senior UAS Assessors (Registered Nurses) for field-based roles in the Bronx, NY area. In this position, you will conduct UAS-NY assessments, develop personalized care plans, and work directly with members to ensure they receive high-quality, person-centered services across the continuum of care.
This role is not remote and requires local travel within selected NYC boroughs. RNs will choose two boroughs to work in. All appointments are pre-scheduled by support staff, ensuring optimized routes and administrative backing.

Must live within commuting distance to the Bronx or surrounding NYC boroughs

Schedule Options
Full-Time: Monday–Friday, 8:30 AM – 5:00 PM (37.5 hrs/week)
Part-Time: 3 days/week (Mon–Wed or Wed–Fri), same hours (22.5 hrs/week)
Average 2–3 assessments/day; all visits scheduled by coordination staff
Field-based | Must reside in or near Bronx, NY

Responsibilities

Perform UAS-NY assessments (initial, reassessment, triggered)

Develop, update, and monitor personalized care plans

Educate members and caregivers on services, conditions, and benefits

Use clinical judgment to identify risks and recommend interventions

Collaborate with care teams and external providers
-
Maintain documentation in compliance with clinical and regulatory standards

Travel to homes or field locations to complete member assessments
Required Qualifications
-
Active & unrestricted NY State RN license
-
2+ years of field-based home health, CHHA, or MLTC experience

UAS experience strongly preferred
-
Strong critical thinking, time management, and independent work skills

Technically proficient – must be comfortable using multiple systems

Must be willing to travel within assigned boroughs

Cannot hold other PRN or per diem roles supporting gov’t-sponsored health plans
