Assistant Clinical Service Director – Bronx, NY
We are seeking an experienced Assistant Clinical Service Director to join a mission-driven long-term care program in the Bronx. In this leadership role, you’ll oversee interdisciplinary teams, support participant outcomes, and serve as a key decision-maker in care delivery across community, day health, and home care settings.
Schedule:

Monday–Friday, 8:30 AM – 5:30 PM
Pay:
-
$125,000–$130,000 DOE

Daily Responsibilities:
-
Provide leadership and oversight of site-based managers and clinical staff (nursing, rehab, nutrition, social work, and home services)

Coordinate care planning meetings, ensure timely follow-ups, and mediate care-related issues

Oversee development and execution of participant care plans to improve satisfaction and outcomes
-
Monitor staffing levels, assignments, and IDT resource allocation

Ensure quality assurance, regulatory compliance, and timely documentation
Required Qualifications:
-
Clear and active NY Registered Nursing license
-
3+ years of administrative experience in a management capacity in CHHA or LTHHCP
-
3+ years of supervisory experience
