Field Case Manager / Registered Nurse
HealthCare Support is actively seeking a Field Case Manager / Registered Nurse to fill an opening with a first class healthcare company delivering exceptional care in Minneapolis, MN and surrounding areas.
Daily Responsibilities for Field Case Manager / Registered Nurse:
- Conduct home visits, assess needs and develop service delivery plan in compliance with guidelines.
- Managing and staying current to assigned caseload.
- Shift: Monday – Friday, 8 a.m. – 4 p.m
Qualifications for Field Case Manager / Registered Nurse:
- 1+ years of experience as an RN
- Prior experience in home health or field-based role
- Have a drivers license and a clean driving record
