RN Case Manager
HealthCare Support is actively seeking a RN Case Manager to fill an opening with a prestigious Healthcare Organization in Deltona, FL.
Daily Responsibilities for RN Case Manager:
Principal Duties and Responsibilities:
- Conduct comprehensive patient assessments to identify medical, psychosocial, and functional needs.
- Develop, implement, and monitor individualized care plans to improve patient outcomes.
- Coordinate care across multidisciplinary teams, providers, and community resources.
- Provide patient and family education on disease management, medications, and preventive care.
- Monitor patient progress, evaluate treatment effectiveness, and adjust care plans as needed.
- Facilitate transitions of care, including hospital discharge planning and follow-up.
- M-F: 8a-5p
Required Qualifications for RN Case Manager:
- Clear and active FL RN license
- 1-2 years of experience
Preferred Qualifications for RN Case Manager:
- At least 1-2+ years of experience
- Experience in meeting the medical needs of patients
- Understanding of complex, general and Chronic Illness
- Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $42.00 – $46.00 an hour
