HealthCare Support is actively seeking a RN Case Manager to fill an opening with a prestigious Healthcare Organization in Deltona, FL.

Daily Responsibilities for RN Case Manager:
Principal Duties and Responsibilities:

  • Conduct comprehensive patient assessments to identify medical, psychosocial, and functional needs.
  • Develop, implement, and monitor individualized care plans to improve patient outcomes.
  • Coordinate care across multidisciplinary teams, providers, and community resources.
  • Provide patient and family education on disease management, medications, and preventive care.
  • Monitor patient progress, evaluate treatment effectiveness, and adjust care plans as needed.
  • Facilitate transitions of care, including hospital discharge planning and follow-up.
  • M-F: 8a-5p

Required Qualifications for RN Case Manager:

  • Clear and active FL RN license
  • 1-2 years of experience 

Preferred Qualifications for RN Case Manager:

  • At least 1-2+ years of experience
  • Experience in meeting the medical needs of patients
  • Understanding of complex, general and Chronic Illness
  • Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $42.00 – $46.00 an hour
