HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with one of the most distinguished healthcare organizations in the US located in Hamilton Ohio 45011.
Daily Responsibilities for Nurse Practitioner:
- Conduct comprehensive assessments, patient facing
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
- Will travel to Butler and Hamilton counties in Ohio during shift
- Will be traveling between 5 facilities: Majestic Care of Fairfield LLC, Home at Taylor’s Pointe, The Laurels of Hamilton, The Laurels of Middletown and Home at Hearthstone
- Shift: 8:00 AM to 5:00 PM
Required Qualifications for Nurse Practitioner:
- Active and unrestricted license in the state which you reside, or ability to obtain by start date
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Minimum 1 year NP experience
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
- Family Nurse Practitioner
- Adult Nurse Practitioner
- Gerontology Nurse Practitioner
- Adult-Gerontology Acute Care Nurse Practitioner
- Covid vaccination
- DEA License
Preferred Qualifications for Nurse Practitioner:
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $70.00 – $80.00 /Hour
