HealthCare Support is actively seeking a UAS Assessment Nurse to fill an opening with a large MLTC in Queens, NY!
UAS Assessment Nurse Duties
- Evaluate patients’ physical, mental, and social conditions through in-person or telehealth interviews.
- Document assessment findings accurately in the UAS-NY system in compliance with state regulations and organizational policies.
- 100% Field I Monday-Friday, 8:30 am – 5:30 pm
Required Qualifications for UAS Assessment Nurse
- BS in Nursing or other health-related area, preferred
- Active and unrestricted New York State RN license.
- 2+ years of experience performing UAS – Uniform Assessment System of New York (UAS-NY)
- 3+ years in MLTC, LTHHCP, MAP, PACE, or other managed care programs
- 5+ years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning

Pay Details:
$110,000 – $115,000 + annual bonus
