UAS Assessment Nurse
HealthCare Support is actively seeking a UAS Assessment Nurse to fill an opening with a leading healthcare organization in the Manhattan, NY.
Daily Responsibilities for UAS Accessor:
- Conduct in-home assessments utilizing the UAS-N to determine initial or continued pan eligibility by the requirement need of Community Based Long Term Care (CBLTC) Services for greater than 120 days, and care needs
- Review the patient's medical history, including current medications, allergies, and relevant medical conditions
- Schedule: Monday – Friday 8:30 – 5:30
Required Qualifications for UAS Assessor:
- NY State RN License
- Recent experience performing UAS assessments
- 3+ years in MLTC, LTHHCP, MAP, PACE, or other managed care programs
- 5+ years of experience in care/case management, disease management, population health management, utilization review, quality assurance, or discharge planning
- Knowledge of community-based long-term care services to dual eligible and frail adult population
Preferred Qualifications for UAS Assessor:
- Bilingual
- BSN preferred
- Prior field experience

Pay Details: $110,000 – $115,000 Salary + Annual Bonus
