HealthCare Support is actively seeking a Transitional Care Nurse to fill an opening with a top Managed Care Company in Rancho Cucamonga, CA. This is a hybrid role which required being in office 2-3 days a week, remote the remaining days – Rancho Cucamonga, CA 91730.
6 month contract – possibility to convert/apply to internal positions once contract is complete
Daily Responsibilities for Transitional Care Nurse:
- Conduct Integrated Transition Care discharge risk assessment for high risk/ high acuity Members.
- Oversee and perform concurrent and retrospective reviews for medical necessity per evidenced based criteria, appropriateness of service and level of care, either through Telephonic review, clinical documentation submitted by respective facilities and/or electronic medical records (EMR) access.
- Conduct reviews to ensure Member’s treatment plan is consistent with Diagnosis(es), specifically initial review within twenty-four (24) hours to ensure Members meet specified criteria for the respective admission(s).
- M-F, 8-5 (Mon & Fri – remote, Tues – Thurs onsite in Rancho Cucamonga, CA)
Required Qualifications for Transitional Care Nurse:
- Possession of an active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN
- 2+ years of Utilization Management / Case Management in a health care delivery setting in Acute or Skilled nursing
- Experience in Concurrent Review and Utilization Management
- Must have a valid California Driver’s license and valid automobile insurance
Preferred Qualifications for Transitional Care Nurse:
- Managed Care experience
- Case Management experience
- Bilingual in Spanish

Pay Details: $43.87 an hour
