LCSW/LMFT Transitional Care Case Manager
HealthCare Support is actively seeking a LCSW/LMFT Transitional Care Case Manager to fill an opening with a Managed Care Organization in Rancho Cucamonga, CA.
Daily Responsibilities for LCSW/LMFT Transitional Care Case Manager:
- Provide services to all Medi-Cal Members transitioning to and from hospitals, institutions, other acute care facilities, and skilled nursing facilities to home or community-based settings
- Assist with coordination of care, provided resource linkages, and collaborate with other Team Members
- Shift: Monday to Friday, 8am to 5pm/ Work Location: 2-3 days Onsite in Rancho Cucamonga, CA and 2-3 days from home
Required Qualifications for LCSW/LMFT Transitional Care Case Manager:
- Master of Social Work and active CA LCSW or LMFT License
- 3-5 years of behavioral health experience
- Experience in motivational interviewing and/or other evidenced-based communication strategies
Preferred Qualifications for LCSW/LMFT Transitional Care Case Manager:
- MCO and/or Case Management experience
- Bilingual in Spanish

Pay Details: $43.87 – $43.87 an hour
