HealthCare Support is actively seeking a Hybrid RN/LPN Patient Care Advocate to fill an opening with a Healthcare Company in Pittsburgh and Philadelphia, PA.

Daily Responsibilities for Hybrid RN/LPN Patient Care Advocate:

  • The Hybrid RN/LPN Patient Care Advocate will be working with members to ensure care gaps are closed, barriers to care are removed, and improve member and provider experiences.
  • The Hybrid RN/LPN Patient Care Advocate will handle telephonic outreach to members, addressing gaps in care, barriers to care, i.e., transportation, offering in-home test kits for gap closures.
  • The Hybrid RN/LPN Patient Care Advocate will also be piloting the Member retention program: addressing member concerns and identify solutions to aid in the improvement of the member experience.
  • The Hybrid RN/LPN Patient Care Advocate will collaborate with other team to solve member concerns
  • This will be a Contract Role (potential to extend)
  • Shift: M-F, 8:30AM to 5:30PM EST
  • Hybrid in Pittsburgh or Philadelphia, PA

Required Qualifications for Hybrid RN/LPN Patient Care Advocate:

  • Graduate from an accredited nursing school
  • Must have RN/LPN license for PA
  • Active and Valid Driver's license
  • 2+ years of experience in patient outreach/health education within case management from another Managed Care Organization
  • Motivational Interviewing skills
  • Understanding of HEDIS measures and quality improvement initiatives
  • MS Office, Smartsheet, Teams, and Zoom experience
  • Good tenure in previous role
  • WFH capability – smart phone, private workspace, high speed internet
  • Soft skills: 
    • Strong communication skills
    • Organizational skills
  • Preferred but required:
    • Project management/lead

Pay Details: $25.00/hour – $34.00/hour
