HealthCare Support is actively seeking an RN Field Case Manager to fill an opening with a prestigious Healthcare Organization in Bronx, NY.

Daily Responsibilities for RN Field Case Manager:

  • Ensure accurate and complete documentation.
  • Communicate and collaborate with the interdisciplinary care team
  • Assist the provider/team with various care coordination activities
  • Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
  • Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition
  • Assist the provider / team with therapy coordination
  • Shift: Monday – Friday 8am to 5pm

Required Qualifications for RN Field Case Manager:

  • Clear and active NY RN license
  • Certified Basic Life Support
  • Full COVID-19 vaccination

Preferred Qualifications for RN Field Case Manager:

  • 1+ year of clinical experience in practice (long-term care setting preferred)
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including the ability to document medical information with written and electronic medical records
  • Effective Communication skills and time management
  • Ability to work with diverse care teams in a variety of settings
  • Access to reliable transportation
  • Ability to travel in between facilities

Pay Details: $55.00 – $65.00 an hour ++
