Field RN Clinical Manager

HealthCare Support is actively seeking a Field RN Clinical Manager to fill an opening with Managed Care partnered SNF in Gettysburg, PA.

Daily Responsibilities for Field RN Clinical Manager:

• Assist the provider/team with various care coordination activities at 6 SNF facilities  between Adams and Cumberland : to prevent unnecessary hospital readmissions for acute and chronic conditions
• Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Mon – Fri 8am-5pm
• 3-6 Month Planned conversion to FTE. FTE Conversions to be reviewed after 6 months. 

Required Qualifications for Field RN Clinical Manager:

• Graduate of an accredited Bachelor of Science Nursing program
• Active PA RN Licensure
• Covid Vaccination

Preferred Qualifications for Field RN Clinical Manager:

• Doctor of Nursing Practice
• Meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Advanced Illness and end of life discussions

Pay Details: $48  – $55 an hour
