Remote RN Case Manager
HealthCare Support is seeking a Registered Nurse to fill an opening as a Case Manager with a major healthcare organization in Oregon.
Daily Responsibilities for the Case Manager:
- Assessing, planning, coordinating, and monitoring services to meet clients’ health and human service needs, emphasizing advocacy, communication, and resource management to achieve quality, cost-effective outcomes. Case Managers enhance patients’ quality of life by developing individualized care plans, coordinating care across providers and resources, and ensuring safe, efficient, and client-centered interventions while adhering to ethical and regulatory standards.
- Shift –
- Monday – Friday 8:00 am – 5:00 pm
- Tuesday – Saturday 8:00 am – 5:00 pm
- Sunday – Thursday 8:00 am – 5:00 pm
Required Qualifications for the Case Manager:
- Clear and active RN license in Oregon
- 3+ years of clinical RN experience
- 1+ years of Case Management
