Pediatric Senior Care Navigator – Hybrid
HealthCare Support is actively seeking a Pediatric Senior Care Navigator – Hybrid to fill an opening with a Fortune top 25 Managed Care Company in Florida. We are looking for candidates only in the below counties.
- Region D: Hillsborough, Manatee, Polk, Hardee, Highlands
- Region E: Seminole, Orange, Osceola, Brevard
- Region I: Monroe but most likely just Miami-Dade
Daily Responsibilities for Pediatric Senior Care Navigator – Hybrid:
- Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes.
- Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on services and benefit options available to improve health care access and receive appropriate high-quality care through advocacy and care coordination.
- Ability to work M-F 8am-5pm
Required Qualifications for Pediatric Senior Care Navigator – Hybrid:
- Active LPN License in FL
- Graduate from an accredited school OR Bachelors degree
- 4+ years of clinical pediatric experience
- 1+ years of case management experience
- Must have a Valid and Active Driver’s license, reliable transportation and insurance
- Excellent documentation experience
- Must be organized with great time management with critical thinking skills
Preferred Qualifications for Pediatric Senior Care Navigator – Hybrid:
- working with medically fragile children
- case management experience

Pay Details: $26.50 to $47.59 an hour
