HealthCare Support is actively seeking a Pediatric RN Case Manager – Hybrid to fill an opening with a fortune 25 Healthcare company.
Daily Responsibilities for Pediatric LCSW/LMHC Case Manager – Hybrid
- Job Title: Pediatric LCSW/LMHC Case Manager – Hybrid
- Company Title: Clinical – Care Manager
- Location: Region B4 – Clay (1st), Duval (2nd), Volusia (3rd)
- Schedule: M–F, 8am–5pm EST | No OT | Training is 4 weeks (first 3 virtual, last week shadowing)
- Start Date: 5/12 – must clear background by 2nd or 4th Monday of the month
- Interviews: Video
- Assignment Type: Right -to-Hire (RTH), starts as a 3-month contract
Day to Day Duties:
- 200 weighted members (mostly Tier 3: higher acuity, more frequent visits)
- Visits can be short and flexible (home, park, office – HIPAA compliant)
- Role includes creating assessments/care plans, coordinating resources, and checking in on progress
Requirements: Pediatric LCSW/LMHC Case Manager – Hybrid
- Must have Master’s degree
- 2+ years of PEDS case management experience – managers will DQ anything less
- Must be a LCSW or a LMHC active license in FL
- Must have a Valid and Active Driver’s license
- Must have MS Office exp – excel, outlook, word

Pay Details: $40 – 41 an hour
