Pediatric LCSW/LMHC Case Manager – Hybrid
HealthCare Support is actively seeking a Pediatric LCSW/LMHC Case Manager – Hybrid to fill an opening with a Managed Care Organization in Miami Dade County.
Daily Responsibilities:
- Assessments are Needs Based – not clinical and not diagnosing the members/providing treatments. EX: linking to providers, referrals for equipment, community recourses, groceries, tutoring
- Calls on average are under 30 minutes long. Candidates may have worked other positions that the calls are 45-60 minutes long but that is not the case for this.
- Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective care.
Required Qualifications for Job Title:
- Must have Master's degree
- 2+ years of PEDS case management experience
- Must be a LCSW or a LMHC active license in FL
- WFH capability – smart phone, private work space, high speed internet
- Must have a Valid and Active Driver’s license
- Must have MS Office exp – excel, outlook, word

Pay Details: $26.50 – 41 per hour
