Pediatric LCSW/LMHC Case Manager – Hybrid
HealthCare Support is actively seeking a Pediatric LCSW/LMHC Case Manager – Hybrid to fill an opening in Hillsborough, Hardee, Highlands, Manatee or Polk counties with a managed care organization.
Daily Responsibilities for Pediatric LCSW/LMHC Case Manager – Hybrid:
- Develop, assess, and adjust, as necessary, the care plan and promote desired outcome
- Assess the member's current health status, resource utilization, past and present treatment plan and services, prognosis, short- and long-term goals, treatment and provider options
- Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients
- Develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member's needs
- Provide patient and provider education
- Location: Hillsborough, Hardee, Highlands, Manatee or Polk counties
- Shift: Mon- Fri 8am- 5pm EST, – No OT required
- Training: M-F 8am-5pm EST for 4 weeks and is not flexible
- Will be 4 weeks long – 1st 2 weeks will be instructor led online, then next 2 weeks will be shadowing experiences
Required Qualifications for Pediatric LCSW/LMHC Case Manager – Hybrid:
- Must have Master's degree
- 2+ years of PEDIATRICS case management experience
- Must be a LCSW or a LMHC active license in FL
- Must have a Valid and Active Driver’s license
- Must have MS Office exp – excel, outlook, word
- WFH capability – smart phone, private work space, high speed internet

Pay Details: $26.50-41/hr
