Pediatric Community Resource Specialist – Hybrid

Pediatric Community Resource Specialist – Hybrid HealthCare Support is actively seeking a Pediatric Community Resource Specialist – Hybrid to fill an opening with a Fortune 100 company. 

Daily Responsibilities for Pediatric Community Resource Specialist – Hybrid:

• This position will be field-based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and providing community resources to address social determinants of health needs
• Caseload is 100 members – members from Child Welfare Specialty Plan who include children in foster care, children who have been adopted from foster care, and young adults in extended foster care 
• this position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan for each member 
• Compliance with care management policies/procedures including outreach and documentation expectation

Required Qualifications for Pediatric Community Resource Specialist – Hybrid:

• Must have a bachelor’s degree 
• 1+ years of pediatric case management experience 
• Experience connecting families/children to local resources to various services including employment, housing, independent living, justice, foster care, etc.
• Valid and Active Driver’s license, reliable transportation, and insurance
• Excellent documentation experience 
• Knowledge of MCO
• Must have MS Office experience – excel, outlook, word
• WFH capability – smartphone, private workspace, high-speed internet

Pay Details: $29.00 – 30.00/ per hour.
