Pediatric Community Resource Specialist
HealthCare Support is actively seeking a Pediatric Community Resource Specialist to fill an opening with Sunshine State Health Plan in Region D of Florida.
Daily Responsibilities for Pediatric Community Resource Specialist:
- This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan provider, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs
- case load is 100 members – members from Child Welfare Specialty Plan who include children in foster care, children who have been adopted from foster care, and young adults in extended foster care – age range through 21 years old.
- This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member compliance with care management policies/procedures including outreach and documentation expectation individuals will be audited on a monthly basis to ensure charts are in compliance
Required Qualifications for Pediatric Community Resource Specialist:
- Must have bachelor’s degree
- 1+ years of pediatric case management experience
- Experience connecting families/children to local resources to various services including employment, housing, independent living, justice, foster care, etc
- Must have a Valid and Active Driver’s license, reliable transportation and insurance
- Excellent documentation experience
- Must have MS Office exp – excel, outlook, word

Pay Details: 30 an hour, paid weekly on W2
