Clinical -Pediatric Community Resource Specialist – Hybrid

HealthCare Support is actively seeking a Clinical -Pediatric Community Resource Specialist – Hybrid to fill an opening with a Managed Care Organization in Miami, FL.

Daily Responsibilities for Clinical -Pediatric Community Resource Specialist – Hybrid:

• This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan provider, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs
• Case load is 100 members – members from Child Welfare Specialty Plan who include children in foster care, children who have been adopted from foster care, and young adults in extended foster care
• This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member
• Compliance with care management policies/procedures including outreach and documentation expectation
• Shift: Mon- Fri 8am- 5pm, 1 hour lunch  – No OT required – weekend work maybe needed during hurricane season for disaster outreach
• Hybrid Set Up:
  • will work from home and be required in the field
  • field visits could include home visits, visiting members in group home/hospital or attending community/agency meetings
     

Required Qualifications for Clinical -Pediatric Community Resource Specialist – Hybrid:

• Must have bachelor's degree
• 1+ years of pediatric case management experience
• Experience connecting families/children to local resources to various services including employment, housing, independent living, justice, foster care, etc
• Must have a Valid and Active Driver’s license, reliable transportation and insurance
• Excellent documentation experience
• Knowledge of MCO

Preferred Qualifications for Clinical -Pediatric Community Resource Specialist – Hybrid:

• coming from a MCO
• foster/care adoption experience

Pay Details: $29/hour – $30/hour
