HealthCare Support is actively seeking a Nurse Practitioner / Physician Assistant to fill an opening with one of the most distinguished healthcare organization organizations in the US located in New York New York 10002.
Daily Responsibilities for Nurse Practitioner:
- Each NPS will be assigned a site where they will see all participants.
- Supervises and manages Nurse Practitioners assigned to their location and assists with the oversight of other Diagnostic Treatment Center’s clinical staff as needed.
- Complete direct reports and annual performance appraisals and collaborate with other DTC Managers providing annual performance feedback to the DTC clinical staff as needed.
In the absence of, or at the direction of, the Medical Director, the NPS will represent or serve as the Medical Director role at their location. - At a minimum, perform annual wellness assessments on all assigned site participants and ensure reflects an understanding of the complexity and multiple comorbidities of the frail and/or elderly. For participants with higher levels of risk, these assessments will be performed more frequently to appropriately address his/her level of need, as described below.
- Provides urgent care services to participants and facilitates the provision of same-day access.
- Assesses and manages participants with acute changes in condition in a timely manner.
- Assesses participants upon return from the hospital/Emergency Department within 5 days. This includes the reconciliation of medications and treatment plans and coordination with the participant’s CPPCP and IDT members to ensure the timely transition of care follow-up visits.
- Reviews the patient’s past medical history at least every 6 months and formulate a comprehensive and complete diagnostic list of current and past medical conditions using clinical knowledge and judgment and the findings of his/her assessment.
- The NPS is responsible for ensuring that all such documentation is complete and accurate, and specific diagnosis codes will be documented in clinical platform and as encounter data as required.
- Participates in all clinical documentation improvement activities with the goal being an accurate and full capture of the burden of illness of our participants and appropriate risk adjustment. Provide feedback and counseling to DTC and IDT staff regarding their clinical documentation as needed
Required Qualifications for Nurse Practitioner:
- Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED
- Minimum of 3 years experience in at least one of these fields: Geriatric, Primary Care, Internal Medicine experience
- Licensed to practice as a Nurse Practitioner in NY required. (Family NP, Geriatric NP, Acute NP) or PA
- DEA Certification required. (will be writing prescriptions)
- AANP or ANCC Board Certification required.
- CAQH profile Required
- Medicare & Medicaid IDs are a must, willing to do homecare
- Individuals must be able to sustain specific physical requirements essential to the job. This includes, but is not limited to: Standing, Sitting/Stationary positions, Lifting/Push/Pull, Bending/Squatting, Stairs/Steps/Walking/Climbing, Agility/Fine Motor Skills, Sight/Visual Requirements, Audio Hearing and Motor Skills (language) Requirements, Cognitive Ability
- A vehicle is required- most patients should be able to be seen in the PACE center but if not- may require a home visit. Mileage, etc is reimbursed
- Can work 8:30am-5:00pm – 1 hour lunch

Pay Details: $70.00 – $92.00 /Hour
