Nurse Practitioner – Float
HealthCare Support is actively seeking a Nurse Practitioner – Float to fill an opening with a Fortune 500 Healthcare Organization in Long Branch, NJ.
Daily Responsibilities for Nurse Practitioner – Float:
Principal Duties and Responsibilities:
- Conduct comprehensive assessments, patient facing
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
- M-F: 8a-5p
Required Qualifications for Nurse Practitioner – Float:
- Clear and active NJ NP license
- 1-2 years of experience
Preferred Qualifications for Nurse Practitioner – Float:
- At least 1-2+ years of experience
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $85.00 – $90.00 an hour
