HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with a leading fortune 500 managed care organization in Bellefonte, PA

Daily Responsibilities for Nurse Practitioner:

• Duties: Conduct comprehensive assessments; Effectively manage acute and chronic medical and behavioral conditions; Ensure accurate and complete documentation, including ICD 10 conditions; Communicate and collaborate with the interdisciplinary care team; Conduct advanced illness and advanced care planning conversations; Provide patients and caregivers with counseling and education
• Schedule: Monday – Friday 8am to 5pm (open to part-time/alternative schedules)
• Location: Bellefonte, PA

Required Qualifications for Nurse Practitioner:

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• PA State NP license
• Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner
• Current, active DEA licensure/prescriptive authority
• Access to reliable transportation
• Full COVID-19 vaccination is an essential requirement of this role
   

Preferred Qualifications for Nurse Practitioner:

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness

Pay Details: $70 per hour – $80 per hour
