HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with a leading healthcare organization serving the geriatric population in Connecticut.

Daily Responsibilities for Nurse Practitioner:

• : The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team.
• Conduct comprehensive assessments, patient facing; Effectively manage acute and chronic medical and behavioral conditions; Ensure accurate and complete documentation, including ICD 10 conditions; Communicate and collaborate with the interdisciplinary care team; Conduct advanced illness and advanced care planning conversations; Provide patients and caregivers with counseling and education
• Location: Field; Newington, CT;  New Haven and Hartford Counties
• Schedule: Monday – Friday 8AM to 5PM (open to part-time/alternative schedules)
   

Required Qualifications for Nurse Practitioner:

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Minimum 1 year NP experience
• Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center

Preferred Qualifications for Nurse Practitioner:

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness

Pay Details: $70.00 per hour – $80.00 hour
