HealthCare Support seeks an LPN Patient Care Advocate to fill an opening at a Fortune Top 25 Managed Care Company.

Daily Responsibilities for an LPN Patient Care Advocate

  • Location: DFH Newark DE Office, Provider offices in Newark DE and surrounding areas, remote
  • Outreach to members and providers to close care gaps and improve experience
  • Partner with providers to improve HEDIS measures and deliver coding education
  • Support quality improvement audits and compliance efforts
  • Schedule appointments, educate members on needed care, and connect to resources
  • Maintain confidentiality of health information

Metrics:

  • HEDIS gap closures, measure improvement, outreach success, appointment scheduling, and provider satisfaction

Requirements: LPN Patient Care Advocate

  • Graduated from an accredited nursing school or have 4 years of experience in place of a degree
  • Must have an LPN license for DE
  • Active and Valid Driver’s license
  • 2+ years of experience in patient outreach/health education/closing care gaps within case management from another MCO
  • Please include IB/OB call metrics, and if the scheduled appointments
  • Strong communication and interpersonal skills
  • Understanding of Provider Practice Scheduling
  • Problem Solving and Resource Coordination
  • Understanding of HEDIS measures and quality improvement initiatives
  • MS Office, Smartsheet, Teams, and Zoom exp

Location: Field role, within 30 minutes driving distance between Newark and Wilmington, DE
Shift: M-F 8-5

Pay Details: $25-34/hour
Length of Assignment: 6-month contract
