LPN Care Manager

HealthCare Support is actively seeking an LPN Care Manager to fill an opening with a Fortune 500 Healthcare Organization in Kissimmee, FL.

Daily Responsibilities for LPN Care Manager:
Principal Duties and Responsibilities:

• Performing focused assessment of chronic care conditions
• Performing medication reconciliation
• Conducting Motivational Interviewing and Self-Management Goal setting
• Providing patient education using standardized, approved educational materials
• Creating referrals to appropriate agencies and resources
• M-F: 8a-5p

Required Qualifications for LPN Care Manager:

• Clear and active FL LPN license
• 1-2 years of experience 

Preferred Qualifications for LPN Care Manager:

• At least 1-2+ years of experience
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $30.00 – $33.00 an hour
