LPN Care Manager
HealthCare Support is actively seeking an LPN Care Manager to fill an opening with a Fortune 500 Healthcare Organization in Kissimmee, FL.
Daily Responsibilities for LPN Care Manager:
Principal Duties and Responsibilities:
- Performing focused assessment of chronic care conditions
- Performing medication reconciliation
- Conducting Motivational Interviewing and Self-Management Goal setting
- Providing patient education using standardized, approved educational materials
- Creating referrals to appropriate agencies and resources
- M-F: 8a-5p
Required Qualifications for LPN Care Manager:
- Clear and active FL LPN license
- 1-2 years of experience
Preferred Qualifications for LPN Care Manager:
- At least 1-2+ years of experience
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $30.00 – $33.00 an hour
