HIRING: LCSW / LMFT Behavioral Health Case Manager

HealthCare Support is actively seeking a LCSW / LMFT Behavioral Health Case Manager to fill an opening with a largest Medicaid health plan and the largest not-for-profit Medicare-Medicaid plan in the country

Daily Responsibilities for LCSW / LMFT Behavioral Health Case Manager:

  • Foster supportive, collaborative relationships with colleagues and partners while modeling ethical behavior in all interactions with members and team members.
  • Drive continuous quality improvement through participation in initiatives, HEDIS gap closure, and training of team members in behavioral health best practices.
  • Manage a behavioral health caseload by providing evidence-based, member-centered care coordination, crisis intervention, and support during transitions of care to improve health outcomes.
  • Participate in interdisciplinary meetings, maintain accurate documentation, and ensure alignment with regulatory standards while supporting the operational and business needs of IEHP.

Required Qualifications for LCSW / LMFT Behavioral Health Case Manager:

  • Master's of Social Work or related field
  • LCSW or LMFT in CA
  • 3 years of behavioral health experience
  • Prefer MCO but will take clinic, hospital, or other BH provider setting
  • Clinical experience – looking for mental health and substance abuse experience
  • Skills:
  • Experience in motivational interviewing and/or other evidenced-based communication strategies
  • Knowledgeable and skilled in evidenced-based communication such as Motivational Interviewing, or similar empathy-based communication strategies
  • Deep understanding and knowledge of mental health and substance use conditions, including both acute and chronic management
  • Soft skills: patient, time management skills, problem solving skills, self-starter, team player, analytical, excellent communication
  • Computer Skills: Excel, Word, Outlook, PowerPoint, Electronic Medical Management systems (for example: Cisco, MHK/Care Prominence, MediTrac, SuperSearch, and/or Web Portal) Preferred but not a must:
  • MCO experience
  • Case Management experience 
  • Bilingual in Spanish

Other Details:

  • Length of Assignment: 6 month contract – possibility to convert/apply to internal positions once contract is complete
  • Pay Details: $43.87 – $43.87/hr (weekly pay)
  • Shift: on- Fri 8am- 5pm EST
  • Location:  Hybrid – in office Tues/Wed/Thurs, remote Mon/Fri –  Rancho Cucamonga, CA 91730
