Case Manager – Hospital 

HealthCare Support is actively seeking a Hospital Case Manager to fill an opening in Gainesville, FL.

Daily Responsibilities for the Hospital Case Manager:

  • Assesses patients' medical and social needs to provide the right care.
  • Leads communication and care planning between doctors, patients, families, and hospital teams.
  • Ensures care is efficient and meets quality standards while coordinating with insurance and outside agencies.
  • Shift Options: DAYS – 8:00AM – 4:30PM (Rotating Weekends) or EVENINGS – 11:00 AM – 7:30 PM (No weekends) 
  • No On-Call 

Required Qualifications for the Hospital Case Manager:

  • Clear and Active FL RN License
  • Experience as a Hospital Case Manager or LTAC Case Manager (preferred) 
  • **If you do not have Case Management experience, a minimum of 3 years ICU experience and/or strong Medical Surgical experience — you are qualified for this role!

Pay Details:

  • $32.91 – $45.25/hour 
  • Sign On Bonus – $15,000 
  • Shift Differentials Available
