Hospice RN Case Manager / Registered Nurse
HealthCare Support is actively seeking a Hospice RN Case Manager / Registered Nurse to fill an opening with a prestigious Hospice organization in Tennessee. Location – Murfreesboro area
Daily Responsibilities for Hospice RN Case Manager / Registered Nurse:
- This RN Case Manager will visit hospice facilities to provide patient care and educate referral partners, including Nursing Home and LTC RNs.
- The RN Case manager will work alongside an NP, RN team, Social worker, and other case managers to collaborate and promote ongoing care
- Provided wound care, monitored vitals, administered medications, and performed death pronouncements as needed.

Schedule: Monday – Friday, 8:00am – 5:00pm
- On-call one night per week from 5pm-11pm (no weekends)
Required Qualifications for Hospice RN Case Manager / Registered Nurse:
- Active RN License in TN
- At least 2 years of RN experience
- Hospice/Palliative care experience
- Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF

- Annual merit increases – up to 3% based on performance
- Annual bonus up to $3900/year
Pay Details: $74,173 – $84,886 per year (based on experience) + bonus eligibility
