Hospice Field Case Manager / Registered Nurse
Healthcare Support is actively seeking a Hospice Field Case Manager / Registered Nurse to fill a prominent in-home hospice provider, located near Duluth, GA.
Daily Responsibilities for the Hospice Field Case Manager / Registered Nurse:
- Provide compassionate and empathetic end-of-life hospice care to adult patients in their homes
- Assess the physical, psychological, social, and spiritual needs of the patient/family of hospice patients, collaborating with the patient/family, physician, and IDG to establish and implement a plan of care, evaluate interventions, and adjust as needed
- Initiate and maintain communication with the attending physician, Medical Director, IDG, Team Director, Team Manager, and other agencies to coordinate optimal care and resource usage for the patient/family
- Document and maintain up-to-date patient records according to protocol, reflecting the care plan’s problems, interventions, actions, and goals while ensuring coordination with the IDG, managing medications, and scheduling visits for continuity and quality care
Required Qualifications for the Hospice Field Case Manager / Registered Nurse:
- Active and unencumbered Georgia RN licensure
- 3+ years of RN experience
- Must have prior hospice, home health, or strong acute care background with field experience

Pay Details: $75k – $89k annually (depending on experience)
