HealthCare Support is actively seeking a Foster Care Case Manager – Hybrid to fill an opening with a Fortune top 25 Managed Care Company in Sedgwick County Kansas or surrounding counties – Reno, Kingman, Harper, Sumner, Harvey, Butler, Cowley and must be comfy serving Sedgwick- must be willing to travel 50% of the time as this role is member facing in their homes and providers offices
Daily Responsibilities for Foster Care Case Manager – Hybrid:
- Evaluates the needs of the member via phone or in-home visits related to the resources available, and recommends and/or facilitates the care plan/service plan for the best outcome, which may include behavioral health and social determinant needs
- Average case load is ranging about 60 and will fluctuate depending on members and staffing
- Develops ongoing care plans for members with high level acuity and works to identify providers, specialists, and community resources needed for care including mental health and substance use disorders
- Coordinates as appropriate between the member and/or family/caregivers, community resources, and the care provider team to ensure identified services are accessible to members
Required Qualifications for Foster Care Case Manager – Hybrid:
- Clear and active Kansas RN license or LMSW, LCSW, LMFT, LMHC, LPC
- Associates or Bachelor’s in Nursing if a RN or Master’s degree if social worker
- Must have valid and active KS driver’s license with reliable transportation and auto insurance
- Must have 2+ years experience working with Foster Care Youth, Child Welfare, Pediatric Mental Health
Preferred Qualifications for Foster Care Case Manager – Hybrid:
- Behavioral health experience – highly preferred
- Child welfare/ foster care experience – highly preferred
- Truama informed care
- Pediatric experience with mental illness
- BSN or PHD
- Medicaid waiver knowledge, specifically TA waiver highly preferred

Pay Details: $38.00 to $44.00 an hour
