HealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune 500 Healthcare Organization in Portland, OR.

Daily Responsibilities for Field RN Case Manager:
Principal Duties and Responsibilities:

  • Ensure accurate and complete documentation.
  • Communicate and collaborate with the interdisciplinary care team
  • Assist the provider/team with various care coordination activities
  • Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
  • Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition
  • Assist the provider / team with therapy coordination
  • Participate in facility-based meetings
  • Schedule and participate in family conferences, team meetings, and team case presentations
  • M-F: 8a-5p

Required Qualifications for Field RN Case Manager:

  • Clear and active OR RN license
  • 1-2 years of experience 

Preferred Qualifications for Field RN Case Manager:

  • At least 1-2+ years of experience
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of complex and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $55.00 – $57.00 an hour
