HealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune 100 major Healthcare organization in Portland, Oregon.

Daily Responsibilities for Field RN Case Manager:

  • Ensure accurate and complete documentation.
  • Communicate and collaborate with the interdisciplinary care team
  • Assist the provider/team with various care coordination activities
  • Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
  • Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition
  • Assist the provider / team with therapy coordination
  • Participate in facility-based meetings
  • Schedule and participate in family conferences, team meetings, and team case presentations
  • Coordination of training, tracking, and compliance with quality measures
  • Schedule: Monday – Friday 8am – 5pm
  • Contract: 4 – 7 Month Contract

Required Qualifications for Field RN Case Manager:

  • Active and unrestricted license in Oregon
  • Minimum 1 year working in a hospital or Long-Term Care facility as an RN
  • Associate degree or higher in Nursing (RN). Certified in Basic Life Support
  • Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment. Access to reliable transportation.
  • Full COVID-19 vaccination is an essential requirement of this role.

Pay Details: $52.00/hour – $57.00/hour
