Field LTSS Service Coordinator (LPN/LVN, LSW, LCSW, LMSW)
HealthCare Support is actively seeking a Field LTSS Service Coordinator (LPN/LVN, LSW, LCSW, LMSW) to fill an opening with a Fortune top 25 Managed Care Company in Tennessee.
We are seeking candidates in the following locations: Hawkins County, Carter County, Blount County, Hamilton County, Knox County, Putnam/Cumberland Counties, Bedford/Cannon/Warren/Coffee Counties, Davidson County, Hickman County, Jackson/Madison County, and in Shelby County
Daily Responsibilities for Field LTSS Service Coordinator (LPN/LVN, LSW, LCSW, LMSW) Coordinator:
- Responsible for overall management of member’s individual service plan within the scope of position, as required by applicable state law and contract; develops, monitors, and revises the member’s service plan to meet the member’s needs, with the goal of optimizing member health care across the care continuum.
- Responsible for performing telephonic or face-to-face non-clinical assessments using a tool with pre-defined response options for the identification, evaluation, coordination and management of member’s non-clinical long term service and support needs.
Required Qualifications for Field LTSS Service Coordinator (LPN/LVN, LSW, LCSW, LMSW):
- Bachelors Degree required
- LPN/LVN, LSW, LCSW, or LMSW or license
- COVID Vaccine is required
- 2+ years of social work experience with intellectually and developmentally disabled individuals or individuals with chronic illnesses/co-morbidities
Preferred Qualifications for Field LTSS Service Coordinator (LPN/LVN, LSW, LCSW, LMSW):
- MA/MS in Health/Nursing preferred
- STRONG preference for 2 years of Case Management experience
- Prefer experience doing member home visits – travel is required

Pay Details: $30.00 to $35.00 an hour
