Field LTSS Service Coordinator
HealthCare Support is actively seeking a Field LTSS Service Coordinator to fill an opening with a Fortune top Managed Care Company in Fort Myers, FL and surroudning areas. Must be comfortable visiting members in and around Lee County, Collier County, Charlotte County, Sarasota County, and Glades County. Mileage is reimbursed.
This is a W2 contract role set to last through December 2025. There may be opportunity for extension, not guaranteed.
Daily Responsibilities for Field LTSS Service Coordinator:
- Will require 80% field work.
- Responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs.
- Responsible for performing face to face program assessments (using various tools with pre-defined questions) for identification, applying motivational interviewing techniques for evaluations, coordination, and management of an individual s waiver (such as LTSS/IDD), and BH or PH needs.
Required Qualifications for Field LTSS Service Coordinator:
- COVID Vaccine is required
- Bachelors Degree in related field
- 2 years of experience working with LTSS population
- 2 years of case management experience
- 2 years of experience working with a social work agency
- Soft Skills: organized, time management skills, strong interpersonal skills
- Own personal transportation, active DL and auto insurance, ability to pass DMV report
- Strong computer skills, ability to adapt to new computer programs quickly
- WFH ability – smart phone, private work space, high speed internet
Preferred Qualifications for Field LTSS Service Coordinator:
- Bilingual in Spanish/English is preferred
- 701B Certification
- CPR Certification
- MCO experience
- Field experience

Pay Details: $31.00 to $32.00 an hour
