Community Resource Coordinator – Field
Field Certified Community Resource Coordinator
HealthCare Support is actively seeking a Field Certified Community Resource Coordinator to fill an opening with a Managed Care Organization in Polk County (Winter Haven, FL). Community Health Worker (CHW) Certification required.
Daily Responsibilities for Field Certified Community Resource Coordinator:
- Efforts will be tailored round key deliverables for the pilot.
- Opportunities for program development, leveraging best practices and direct feedback on program experience.
- Outcome reporting (i.e. outreach efforts, successes, barriers, etc.)
- Utilization of clinical documentation system
- Member/Provider Outreach (telephonic + face to face)
- Serving as the health plan liaison for new/existing community-based organizations
- Completion of health risk assessments/screenings
- Addressing social barriers (i.e. closed loop referrals)
- This will be a Field position in Polk County, FL (may travel up to 200 miles per day)
- Schedule is Monday to Friday between the hours of 8AM to 6PM
- ThIs is a Contract position for 5 month (potential to extend or convert into a permanent role)
Required Qualifications for Field Certified Community Resource Coordinator:
- High School Diploma or GED
- Must be residing in Polk County
- Community Health Worker Certification required.
- Working Knowledge of Social Determinants of Health (SDOH) barriers
- Working Knowledge of geographic area and known Community Based Organizations
- Working knowledge of motivational interviewing and care plan development/goal setting
- Clear and active Driver’s license, reliable transportation, and car insurance

Pay Details: $25.00/hr
