Community Resource Coordinator – Field

HIRING: Community Resource Coordinator – Field (Field based in Hillsborough County)

HealthCare Support is actively seeking a Community Resource Coordinator to fill an opening with a largest not-for-profit Medi-Cal and Medicare Health Plan in Tampa Florida 33605.

Daily Responsibilities for Community Resource Coordinator – Field:

• Efforts will be tailored round key deliverables for the pilot
  • Opportunities for program development, leveraging best practices and direct feedback on program experience
  • Outcome reporting (i.e. outreach efforts, successes, barriers, etc.)
• Utilization of clinical documentation system
• Member/Provider Outreach (telephonic + face to face)
• Serving as the health plan liaison for new/existing community-based organizations
• Completion of health risk assessments/screenings
• Addressing social barriers (i.e. closed loop referrals)
• Field based in Hillsborough County, will travel up to 200 miles per day (will usually be less, but up to 200)

Required Qualifications for Community Resource Coordinator – Field:

• Located in Hillsborough County
• Bilingual highly preferred
• Community Health Worker Certification – required for Florida 
• Working Knowledge of Social Determinants of Health (SDOH) barriers – (examples: safe housing, violence, education, job opportunities, income, nutritious food, physical activity opportunities, polluted air and water, language and literacy skills
• Working Knowledge of geographic area (i.e. posted county/service area) and known Community Based Organizations – (Examples: Nutrition services, housing assistance, education, substance abuse services, transportation services for healthcare appointments, peer support, medication delivery
• Working knowledge of motivational interviewing and care plan development/goal setting
• General understanding of Microsoft office suite (i.e. work, outlook, excel)
• HS Diploma or GED
• Clear and active Driver’s license, reliable transportation, and car insurance

• 5 months, potential to extend and convert
