Care Coordinator (Field)
Care Coordinator, HealthCare Support seeks a Care Coordinator to fill a Fortune 100 company opening.
Daily Responsibilities for Care Coordinator:
Determine members’ needs and eligibility for various services through field-based collaboration with medical, behavioral, and social support staff. Conduct screening and risk assessment interventions per program guidelines. The care manager will have 3 days of travel for visits to see the members in their homes and 2 days working remotely for documentation of assessments as well as any follow-up and tasks required to work.
Required Qualifications for Care Coordinator:
- 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations with chronic or complex conditions.
- Bachelor’s degree in social work, Health Services, Behavioral Science or related field or LVN with 2 – 4 years of experience
- Microsoft Office and Excel proficient
- Clear and active driver’s license, reliable transportation, and insurance

Pay Details: $25- $26/ per hour.
Location: Hybrid position, 2 days WFH, 3 days in the field
