HealthCare Support is actively seeking a Care Coordinator to fill a remote opening with a Managed Care Organization.

Daily Responsibilities for Care Coordinator:

  • Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
  • May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
  • Shift: Mon- Fri 8am- 5pm CST
  • Remote position – must reside in San Antonio, TX
     

Required Qualifications for Care Coordinator:

  • 2+ years of experience in a managed care environment or working with people who have disabilities and/or with vulnerable populations who have chronic or complex conditions
  • Microsoft Outlook, Microsoft Teams, Zoom, Microsoft Word, Microsoft Excel, SharePoint, Internet and reliable wi-fi
  • HS Diploma or GED

Preferred Qualifications for Care Coordinator:

  • Bachelor’s degree in Social Work, Health Services, Behavioral Science, or related field, or equivalent experience.

Pay Details: $18/hour – $20/hour
