Bilingual Spanish Long Term Care Manager – Hybrid
HealthCare Support is actively seeking a Bilingual Spanish Long Term Care Manager – Hybrid to fill an opening with a Fortune 100 Managed Care Company in Tampa FL.
You must be located around Downtown Tampa, Ybor city, South and East Tampa. We cannot accept other areas such as Riverview, Brandon, Sun City, Ruskin, Apollo beach etc.
Daily Responsibilities for Bilingual Spanish Long Term Care Manager – Hybrid:
- Responsible for members gaining access to needed services through coordination and integration of medical and long-term care services for orientation, care plan development, assessment, and care coordination.
- They will be assessing needs of elderly patients-i.e. asking them questions to see what type of services they need for expr home health care
- Working with the impoverished elderly population (55+ yrs. old).
- M-F 8AM-5PM. Up to 80% field based
Required Qualifications for Bilingual Spanish Long Term Care Manager – Hybrid:
- Must be Bilingual Spanish
- Bachelor’s degree and 2+ years of case management experience
- OR LPN with 4+ years of case management experience
- Must be comfortable going into members’ homes for assessments and able to handle caseloads of 40+ members
- Must have experience with EMR
- Must have a Valid and Active Driver’s license
- Must have MS Office exp – excel, outlook, word
Preferred Qualifications for Bilingual Spanish Long Term Care Manager – Hybrid:
- Knowledge of healthcare and managed care preferred
- Home health, long term care exp or Discharge planning exp
- Exp working for the Department of Children and Families

Pay Details: $24 – $28 an hour
